(Richmond Township, PA) -- An investigation is underway into the death of a worker at a manufacturing plant in Berks County earlier this week. Authorities said 48-year-old Stephen Rapp was killed when he became trapped in a machine at the East Penn Manufacturing Facility in Richmond Township Tuesday night. The Lehigh County Coroner's Office called Rapp's death "a terrible accident." The Occupational Safety and health Administration is investigating.
